If someone was in a lunatic asylum during the 19th century, were they necessarily a lunatic?
or could they be there for some other reason – for example paralysed?
Some may have just been epileptic or even a diabetic. Insulin is a hormone and when it is out of whack a person can just act out and seem a little crazy.

No, most of them wern’t necessarily lunatics. Some ignorant Doctors sent sick people over there just becasuse they couldnt understand what was going on; even homosexuals were send there; sometimes to bring new ideas was a one way ticket to the asylum; many scientifis, writers, painters were send there too.
In other times the asylum was used as place to send enemies for political or personal reasons without having to get into the trouble of having the person killed; all they needed was a corrupt doctor, some friends and some money.
